Please note: These items are required before THAW can begin processing your online application:
  • All sections of the application must be completed.
  • Uploaded copy of the applicant’s valid ID (i.e. driver’s license or state-issued ID)
  • Uploaded copy of the applicant’s Social Security card.
  • Social Security numbers for all household members.
  • Uploaded proof of all household income for the past 60 days.
    • Including: unemployment benefits, Social Security income, pension/retirement benefits, Veterans Affairs benefits, cash assistance (FIP), adoption subsidy/direct care, worker’s compensation, alimony, interest annuities, and dividends.
    • For child support, upload proof for past 90 days of proof
    • If you are self-employed, complete the Self-declaration of Income section. You must provide documentation of this income.
    • If your household has no income, complete the Zero Income Affidavit section.
  • Uploaded copy of the most recent utility bill for which you are seeking assistance.
  • Your final review and signature (bottom of the final page).
